Eligibility Requirements

You are eligible to be a participant in Mercy LIFE if, at the time of enrollment, you:

• Are age 55 or older
• Live in one of the service area zip codes
• Are certified by the state as eligible for nursing facility level of care
• Are able to live safely in the community with services provided by Mercy LIFE

A LIFE enrollee may be, but is not required to be, any or all of the following:

• Entitled to Part A
• Enrolled under Part B
• Eligible for Medicaid

Mercy LIFE will not restrict enrollment based on Medicare or Medicaid eligibility.

Mercy LIFE services are typically free to seniors who qualify for Medicare and Medicaid or Medicaid only. Participants who do not qualify for Medicaid will be charged a monthly fee and have the option to privately pay.
